Caring-on-Call Program

Caring-on-Call provides an advocate and partner in care to participants who have no family, no family in town, or family members who need back-up support. The participant gains peace of mind knowing that they will not be alone in a crisis or a health downturn that requires extra help or arrangements. (Please note that on-call services are already included for regular monthly care management clients.)

Program Description

  • Initial meeting to obtain medical, social and family history. Copies of insurance cards and photograph are obtained.
  • Afterwards, monthly telephone visits to obtain medical, social and care updates to stay current on participant’s healthcare status and needs.
  • Emergency on call: Care Manager meets participant at Emergency Room to advocate on their behalf, communicate for them if they are not able, and to stay in touch with designated friends/family, if desired. Hourly rates apply.
  • Hospital-to-home (or rehab when necessary) services include guidance in selecting rehab facility or home health company and coordination of transition. Services also include ensuring medication changes, hospital orders, adaptive equipment and new specialist appointments are noted and met after discharge. Any special transportation arrangements will be made for the participants. Hourly rates apply.

How do I enroll my loved one in the Caring-on-Call program?

  • Step 1: Call 256-489-7573 to request an enrollment packet. There’s a discount for annual enrollment.
  • Step 2: Our Caring-on-Call program director will schedule an initial meeting to gather important documentation for appropriate monitoring. The one-time enrollment fee is waived for current clients.
